scheme envisages providing numerous
benefits to the members by way of
free medical examinations, a rebate
on Hospital Charges etc. |

| BENEFITS |
|
The
entitlements under benefits are as follows:-
- Out
Patient Clinic: Free examination
by the Scheme Physicians in the Hospital
Consulting rooms during clinic hours.
These hours are subject to change to
suit the general convenience of members.
- Outside
Visit by Scheme Physicians: The
Scheme Physicians can visit outside
the hospital to see Scheme members and
are allowed to charge a maximum of Rs.
250 for each outside visit. However
beyond Mahim the charge is Rs. 300/-.
Night visits between 8 p.m. and 7 a.m.
are 50% extra. Transportation is payable
by scheme members to doctors. A visit
to a subscriber as an in-patient of
the Hospital is considered as an outside
visit. Likewise, medical attention in
the Outpatient department outside clinic
hours is charged as an outside visit.
- Discounts:
A reduction of 20% on all bills pertaining
to the following Hospital Services.
PATHOLOGY, INCLUSIVE OF BLOOD BANK,
HISTOLOGY, PULMONARY & STRESS TEST,
X-RAY, MRI, CT, EEG, SONOGRAPHY, PHYSIOTHERAPY,
ANGIOGRAPHY, ECHOCARDIOGRAPHY, O.T.
CHARGES, ECG & OPD FEES, MAMMOGRAPHY,ENDOSCOPY
, HEALTH CHECKUP AND COLOUR DOPPLER
STUDIES.

| CRITERIA
FOR ELIGIBILITY |
- The
age for new subscribers must be below
75 years.
- Spouses
and children under18 years of age, residing
with the subscriber, are covered under
a specified category. Children above
the age of 18 can however be enrolled
separately as single subscribers.
- There
is a waiting period of one calendar
month for all benefits other than maternity
for which the waiting period is 6 months.
- All
new members must be recommended by an
existing member of the Scheme, a Trustee
or Breach Candy Hospital Senior Staff.
- Subscription
is payable in advance for one year.
- Membership
would be restricted to residents of
Mumbai only.

| PROFESIONAL
FEES FOR CONSULTANTS

Rs. 400 per visit in the Hospital consulting
room
Rs. 400 per visit in Breach Candy Hospital
patient room
Rs. 500 per visit in the hospital consulting
room with ECG
Scheme
Obstetricians and Gynaecologists
Free
Examinations by the Scheme Obstetricians
and Gynaecologists will be carried out
in the Hospital consulting room at the
hospital during advertised clinic hours.
These hours are subject to change from
time to time to suit the general convenience
of members and doctors.
The
following charges may be made by the scheme
Obstetricians/Gynaecologists.
For normal maternity cases inclusive of
all visits: Rs. 4000
For Forceps cases: Rs. 6000
For complicated maternity cases including
Caesarean section
and inclusive of all visits: Rs. 8000
To
register of admission to the maternity
ward, a non-refundable charge of Rs.300
is payable.
Cancer and Allied Diseases
The
cancer check up package will focus on
cancer detection in Women and will encompass
the following tests
-
PAP smear : To detect cancer of the
Cervix of the Uterus
- Mammography
: To detect cancer of the Breast
- Ultrasonogrpahy
(USG) : To detect Ovarian and other
Abdominal Cancers
- Bone
Densitometry : For detection of Osteoporosis
- Chest
X-ray : To detect Lung Cancer
- Gynaecological
consultation
The
special Package price will be Rs 3,000

Category C - Self & Spouse & 1

Category D - Self & Spouse & 2
children: Rs. 1800
A
membership card will be issued to all
members mentioning their Code No. and
category. This should be produced whenever
the member avails of Hospital Services.
